245D Services

Licensed by MN Department of Human Services, our 245D services include: Homemaker, In Home Support with and without training, Night Supervision, Crisis Respite, Respite In and Out of Home, Home Delivered Meals, Parent Training, Positive Supports, and Individual Living Supports.

Homemaker

Our patient caregivers complete cleaning tasks including laundry, clean the kitchen/Dishes, bathroom, bedrooms, living area, take out the trash, as well as vacuum/dust, and make the bed/change bed linens. We understand the importance of feeling comfortable in your own home and therefore go at your pace of what needs to be done.

In Home Supports/Individual Living Supports

These services provide companionship and accessibility to the community by going on walks, accompaniment to appointments, transportation for errands and shopping.


Respite

Respite care provides short-term relief for primary caregivers. It can be arranged for just an afternoon or for several days or weeks. Care can be provided at home (day and night), or out of home (day only).

Positive Supports

Positive Behavior Support (PBS) is a set of research-based strategies used to increase quality of life and decrease problem behavior by teaching new skills and making changes in a person's environment.

Parent Training

Behavioral parent training aims to not only change a child's behaviors but also shape parental behavior. Helping parents apply what they've learned takes practice. During role-playing, parents practice interventions on the behavior therapist; the therapist offers support and feedback along the way.

Night Supervision

Overnight assistance and supervision provided by an awake staff in a person’s own home.
